Do I need a referral?

In most situations, no referral is needed. Michigan is a Direct Access state, allowing you to select your preferred provider without one. However, certain insurance plans may still require a referral from your primary care physician (PCP) or a prescription from a doctor.

We are in-network with the following insurance providers. If your plan is not listed, we offer competitive self-pay options. Additionally, we can provide a super-bill, making it easy for you to submit proof of your visit for potential out-of-network reimbursement.

What should I wear to my appointment?

Wear something comfortable and easy to move in, as you'll be active during your session. Athletic or loose-fitting clothing is best, and if possible, choose attire that allows easy access to the area being treated. For example, if we're working on your knee, avoid wearing jeans.

How can I pay for my appointment?

We accept all major credit and debit cards (Visa, Mastercard, American Express, and Discover), HSA/FSA cards, Venmo, Zelle, checks made out to PhysioFit LLC, and cash.

What is a Superbill?

A superbill is an itemized receipt that includes details about your visit, such as diagnosis and treatment codes, provider information, and the amount paid. If we are out-of-network with your insurance, you can submit a superbill to your insurance company for possible reimbursement. While reimbursement isn’t guaranteed, many plans offer partial coverage for out-of-network services.
